On-line GACC Waiver Form

Click on the link provided below.
Enter you name and e-mail address.
Click the blue “BEGIN SIGNING” tab.
Check the box “I agree to use electronic records and signatures”.
Click the orange tab entitled “GET STARTED”.
Read the document as you scroll to the bottom of the document.
Click on the blue signature box.
Pick on of the six signature fonts by clicking on the one you want to use.
Type your name in the box above the font selections.
Click the blue box at the bottom right entitled “SIGN”.
Click the blue text box to the right of PRINT NAME and type your name.
The blue “Date” entry is automatically populated.
Click the orange “DONE” box at the top right.
Go to your e-mail and open the e-mail from DigiSigner. It could take a few minutes to receive your e-mail message depending on     how your e-mail client is configured.
Click the blue “Verify Email” box. It should now show sucessfully completed and you will receive a completed waiver form in your e-mail. If you don’t receive a copy of the completed form the signing was not complete.
If you received a signed copy of the signed form the process and you are done.
